New Patients

We are always looking to welcome new patients into our office! New patients are required to print out the forms below for completion or can come into the office 15 minutes prior to your appointment to fill them out.

For minors, a legal guardian or parent must be present for their initial appointment to discuss treatment options.

​

Patients that are transitioning into our office from another dentist should sign a release form from their prior dentist and have the dentist email any current X-rays to rwjansendds@gmail.com
